Familial monomelic amyotrophy (Hirayama disease): two brothers with classical flexion induced dynamic changes of the cervical dural sac.

چکیده
Sir, In Hirayama disease or monomelic amyotrophy (MMA), dynamic changes of the cervical dural sac and spinal cord associated with neck flexion have been suggested to cause transient cord compression. This finding has not been reported in familial MMA. Two brothers were clinically evaluated and underwent magnetic resonance imaging (MRI) of the spinal cord. They were born to nonconsanguineous parents and had classical manifestations of MMA of the upper limb. Both demonstrated presence of characteristic flexion induced dynamic changes of the cervical dural sac with enhancing posterior epidural space. This observation among familial MMA has not been reported till date.

Monomelic Amyotrophy (Hirayama Disease) With Upper Motor Neuron Signs: A Case Report
Monomelic amyotrophy (MMA), also known as Hirayama disease, is a sporadic juvenile muscular atrophy in the distal upper extremities. This disorder rarely involves proximal upper extremities and presents minimal sensory symptoms with no upper motor neuron (UMN) signs. It is caused by anterior displacement of the posterior dural sac and compression of the cervical cord during neck flexion. An 18-...
متن کاملNosology of Juvenile Muscular Atrophy of Distal Upper Extremity: From Monomelic Amyotrophy to Hirayama Disease—Indian Perspective
Since its original description by Keizo Hirayama in 1959, "juvenile muscular atrophy of the unilateral upper extremity" has been described under many nomenclatures from the east. Hirayama disease (HD), also interchangeably referred to as monomelic amyotrophy, has been more frequently recognised in the west only in the last two decades. HD presents in adolescence and young adulthood with insidio...
متن کاملHirayama disease: imaging profile of three cases emphasizing the role of flexion MRI.
We report three cases of Hirayama disease, cervical flexion myelopathy presenting as unilateral or bilateral asymmetric muscular atrophy of forearm and hand involving C7 - T1 myotomes in young males. MRI revealed asymmetric cord atrophy, altered signal intensity of cord, posterior dural detachment and enlarged posterior epidural space with multiple flow voids. This article emphasizes the role o...
متن کاملThe diagnostic criteria of benign monomelic amyotrophy.
When we consider the Hirayama disease, a contradiction arises from the point of this criterion. According to De Freitas and Nascimento 4 , when MMA is restricted to the distal aspect of the upper limb, it is known as Hirayama disease. Hirayama disease has been called many different names: MMA, juvenile muscular atrophy of distal upper extremity, juvenile asymmetric segmental spinal muscular atr...
متن کاملA Korean Case of Juvenile Muscular Atrophy of Distal Upper Extremity (Hirayama Disease) with Dynamic Cervical Cord Compression
We present a Korean case of Hirayama disease with its typical neuroradiological findings of forward displacement of cervical dural sac and compression of the lower cervical cord during neck flexion. A 15-yr-old boy was presented with a one-year history of progressive weakness and atrophy affecting bilateral hands and forearms. The electrodiagnostic findings were compatible with the lesion of th...
